Job Description

We’re seeking a primary care physician committed to transforming healthcare for seniors.

• Provide comprehensive care for patients with acute and chronic conditions, managing outcomes and building trust.
• Act as a health coach, setting and following up on personalized health goals.
• Simplify care plans to promote actionable behavior change for patients and team members.
• Lead a team of care providers, coordinate with specialists, and engage with hospital and transitional care teams.
• Monitor key metrics like patient outcomes, experience scores, and cost measures to meet organizational goals.
• Maintain compliance, participate in clinical and quality meetings, and foster a positive center culture.
• Demonstrate leadership, availability, evidence-based practice, and a commitment to quality and self-care.
• Require MD/DO in Family Medicine, Geriatrics, or Internal Medicine, with active licensing and board eligibility or certification.
• Must hold a DEA license and BLS certification within 90 days.
